What Is Medicare Advantage?

Medicare Advantage (Part C) is an alternative way to receive your Medicare Part A and Part B benefits. Instead of receiving these benefits directly through Original Medicare, you enroll in a Medicare Advantage plan offered by a private insurance company approved by Medicare.

Many Medicare Advantage plans include Part D prescription drug coverage, along with additional benefits such as dental, vision, hearing, and fitness programs. Some plans may also have a $0 monthly plan premium, although other Medicare costs may still apply.

The trade-off is that Medicare Advantage plans typically have provider networks, copayments, and other cost-sharing requirements. Plan benefits, costs, provider networks, and drug coverage can also change from year to year.

HMO (Health Maintenance Organization)

HMO Medicare Advantage plans typically require you to choose a primary care provider (PCP) who coordinates your care. Depending on the plan, you may need a referral to see certain specialists.

These plans often offer lower premiums and predictable copayments, but you generally need to use the plan’s network of doctors and hospitals for covered care, except in emergencies and other situations allowed by the plan.

Best for: People who prefer lower-cost coverage and are comfortable using a provider network and coordinating care through a primary care doctor.

PPO (Preferred Provider Organization)

PPO Medicare Advantage plans offer more flexibility in choosing doctors and specialists. You generally don’t need referrals to see specialists, and you may be able to receive covered care from out-of-network providers—usually at a higher cost.

PPO plans may have higher premiums or cost-sharing than some HMO plans, but they can provide greater flexibility when choosing healthcare providers.

Best for: People who value flexibility, want to see specialists without referrals, and are comfortable paying more when using out-of-network providers.

PFFS (Private Fee-for-Service)

PFFS Medicare Advantage plans determine how much the plan will pay for covered healthcare services and what you’ll pay. You generally don’t have to choose a primary care doctor or get referrals to see specialists.

Unlike HMO and PPO plans, PFFS plans may not have a traditional provider network. However, doctors and other providers must accept the plan’s terms and conditions and agree to treat you for covered services.

Best for: People who want more provider flexibility and live in areas where traditional Medicare Advantage networks may be limited.

SNP (Special Needs Plan)

Medicare Advantage Special Needs Plans (SNPs) are designed for people who meet specific eligibility requirements, including certain chronic health conditions, dual eligibility for Medicare and Medicaid, or who live in certain institutions or require institutional-level care.

SNPs tailor their benefits, provider networks, and care coordination to the needs of the people they serve. Availability and eligibility vary by location and plan.

Best for: People who qualify for a Special Needs Plan based on a specific chronic condition, Medicare and Medicaid eligibility, or institutional care needs.

Medicare Advantage plans include an annual Maximum Out-of-Pocket (MOOP) limit for covered services. Once your copays and coinsurance reach that limit, the plan pays 100% of covered medical expenses for the rest of the year. The MOOP does not include prescription drug costs unless specified under Part D rules.
